Term Life Insurance (with AD&D) Optional Benefit

1-4 members: $25,000 coverage, subject to approval.

5 or more members: choose from $25,000, $50,000, $100,000, 1 times annual salary, 2 times annual salary.

Subject to approval. Provides easy term life insurance. Dependent Life Insurance (with AD&D) Optional Benefit

Choose between $5,000 spouse and $2,500 per dependent child, or $10,000 spouse and $5,000 per dependent child. The Dependent Life Insurance option provides basic life insurance coverage for your spouse and dependent children, regardless of how death may occur. Coverage is automatic-issue, regardless of the health status of the spouse or dependent.

Critical Illness Insurance Optional Benefit

Flat $10,000 or 20,000 of Critical Illness coverage, payable after 30 days upon the diagnosis of one of the covered conditions.  Benefit is payable in a lump sum 30 days after diagnosis of a life threatening form of one of the covered conditions.

Short Term Disability Insurance Optional Benefit

This benefit provides you with a regular income to replace income lost because of a short term disability due to illness or injury. This benefit usually begins 1 day after accidents or 8 days after sickness commences and continues until you are no longer disabled as defined by the policy, to a maximum of 17 weeks.

As the Short Term Disability benefit partially mirrors covered provided under Employment Insurance benefits, you may qualify for a discount on your payroll E.I. premiums if you add this coverage.  Many employers do not include Short Term Disability insurance as it duplicates benefits payable under E.I.

Long Term Disability Insurance Optional Benefit

LTD coverage provides you with a regular income to replace income lost because of a lengthy disability due to illness or injury. Benefit begins after the waiting period is over (usually 17 weeks) and continues until you are no longer disabled as defined by the policy. Coverage typically insures 67% of pre-disability income, and remains in force until age 65.
